Extra tax applies to over 26,000 lbs and/or more than 2 axles. Most PT and wide body 'birds will owe the extra tax. Exemption applies only to school busses, government vehicles, and "charitable" (church) busses and similar vehicles. It's pretty clear if you read the stickers on the pumps.
However, the fill-ups I did at Flying J in Kingman (on my way into high-tax CA), the clerk just gave me the discount without my asking -- I did my fillup at the commercial pumps, not the RV pumps. I didn't really check to stickers on the pump, so I assumed the clerk knew what they were doing. On my last visit to the Kingman Flying J, I actually read the pump-sticker and didn't see that I qualified as exempt.
